하지불안증후군에서 인지기능에 대한 프라미펙솔의 효과

Abstract

Objectives:

Neuropsychological evaluations have revealed cognitive dysfunction in restless legs syndrome (RLS) patients, especially frontal lobe functions. Although dopamine agonists improve RLS sensory symptoms and periodic leg movements during sleep, there was no study regarding the effects of dopamine agonist on cognition in patients with RLS.

Methods:

Sixteen drug-naïve RLS patients were enrolled. They underwent sleep questionnaires, Beck Depression Inventory, and neuropsychological evaluation, consisting of Rey Complex Figure Test and recognition trial (RCFT), Korean-California Verbal Learning Test, Controlled Oral Word Association Test (COWAT) before and after treatment with pramipexole (PMP). PMP was administered over a period of 12 weeks every night 1 h before bedtime. Paired Student’s t-test and Spearman’s correlation analysis were used for analyses.

Results:

After treatment with PMP, the International RLS Severity Scale score was significantly decreased. Sleep quality and depression also significantly improved after the treatment. RCFT immediate recall, COWAT phoneme word frequency, and digit symbol coding scores significantly improved compared to those before the treatment (17.72±7.84 vs. 20.13±7.33, p=0.05; 36.50±12.62 vs. 41.50±12.58, p=0.03; 68.19±21.11 vs. 73.81±21.11, p=0.02, respectively). There were no significant correlations between changes in clinical variables and in cognitive functions before and after the treatment.

Conclusions:

This study suggests that treatment with PMP in RLS patients improves not only RLS sensory symptoms, but also cognitive functions.

서 론

하지불안증후군(restless legs syndrome, RLS)은 다리를 움직이고 싶은 견딜 수 없는 충동과 함께 다리에 매우 불편하고 불쾌한 감각증상이 동반되는 감각운동 신경질환이다. RLS는 불편한 감각증상과 연관되어 수면장애 및 주간 업무장애, 우울증을 흔히 동반한다. 최근 RLS 환자에서 인지기능 저하 를 보고한 연구들이 있으며, 특히 앞이마엽 영역에서 기능저 하가 두드러지는 것으로 보고하였다.1-3
Restless legs syndrome 환자에서 인지기능 저하를 처음 보고한 연구에서는, 환자들에서 앞이마엽 영역에 해당하는 인지기능 저하를 보이고 이는 수면박탈 후 보이는 인지기능의 저하와 유사한 것으로 나타나, RLS에서의 인지기능 저하가 만성수면부족에 이차적으로 나타나는 것으로 생각하였다.4 그러나, RLS 환자와 같은 정도로 수면을 박탈한 대조군과의 비교연구에서는 대조군에서 RLS 환자군보다 인지기능이 오히려 저하된 결과를 보였다. 이는 수면부족이 인지기능에 영향을 미치지만, RLS 환자에서는 이를 보상하는 기전이 작동한 것으로 볼 수 있다.2
수면부족 이외에도 RLS 증상으로 인한 주의력 저하, 동반된 우울증 및 대뇌피질 자체의 기능 이상이 RLS 환자의 인지기능에 영향을 미칠 수 있다. 사건유발전위 연구에서는 RLS 환자가 정상인에 비해 P300 진폭이 감소하고 잠복기가 연장 되어 있었으며, 이러한 변화는 주로 주의력 저하 및 대뇌 피질 자체의 기능 이상과 연관이 있는 것으로 나타났다.5
도파민작용제(dopamine agonist)는 중증도 이상의 RLS환자에서 일차적으로 추천되는 약제이다.6 도파민작용제는 RLS의 일차적인 감각 증상을 호전시키며, 이외에도 수면의 질, 수면 중 주기적사지운동, 우울증상, 삶의 질 등을 호전시키는데 이는 RLS 중증도의 호전 정도와 연관이 있었다.7-11 이처럼 도파민작용제가 RLS와 관련된 다양한 증상들을 호전시키는 것으로 알려져 있지만, 아직까지 인지기능에 어떤 영향을 미치는지 연구한 논문은 없다.
본 연구는 RLS 환자에서 도파민작용제를 투여한 후 인지기능의 호전이 나타나는지 확인하고자 한다. 그리고 인지기능이 호전된다면, 호전되는 임상적인 요인이 무엇인지 알아 보고자 하였다.

방 법

대 상

이전에 한 번도 치료받지 않은 16명의 RLS 환자들이 연구에 포함되었다. 모든 환자들이 정형화된 수면 설문지를 통한 질문에 응답했고 신경학적 검사를 시행 받았다. RLS는 International RLS study group에서 제안한 기준에 따라 진단하였으며,11 제외 기준은 다음과 같다: 1) 이차성 RLS를 유발할 수 있는 동반질환의 존재: 임신, 만성신장질환, 말초신경질환, RLS 유발 가능 약물의 복용력(예, 신경이완제, 항우울증약, 항히스타민제 등); 2) 수면무호흡증, 렘수면행동장애, 사건수면 등의 수면질환; 3) 인지기능장애로 인하여 증상을 정확히 호소할 수 없는 경우; 4) 주의력부족과잉행동장애, 본 태성 떨림, 파킨슨병, 신경이완제에 의한 정좌불능증, 울혈성 심부전, 혈관 및 신경성 파행, 척수병증, 관절염 등 RLS 유사 증상을 보일 수 있는 질환.
수면 습관과 약물 복용력을 문진하였으며, Global Sleep Assessment Questionnaire, Pittsburgh Sleep Quality Index (PSQI), Epworth Sleepiness Scale(ESS), Insomnia Severity Index(ISI) 수면 설문지를 함께 시행했다.12-15 우울정도는 Beck Depression Inventory(BDI) 설문지를 통해 평가하였으며,16 RLS의 중증도는 International RLS Severity Scale(IRLS)을 통해 평가하였다.17
모든 환자에서 혈색소, 혈당, 혈청 크레아티닌, 철/페리틴, 갑상선호르몬 수치는 정상이었다. 모든 환자에서 문서화된 사전동의를 받았고, 연구계획서는 기관윤리심의위원회(Institutional Review Board) 승인을 받았다.

신경심리검사

신경심리검사는 집중력을 확인하기 위한 숫자외우기 검사(digit span test), 언어기능을 평가하기 위한 한국판 보스톤 이름대기 검사 단축형(Short form of the Korean-Boston Naming Test, S-K-BNT), 시공간 구성능력과 기억력을 측정하기 위한 레이-오스터리에스 복합도형 검사(Rey-Osterrieth Complex Figure Test, RCFT), 언어 기억 및 학습을 평가하기 위한 한국판 캘리포니아 언어학습검사(Korean-California Verbal Learning Test, K-CVLT)를 시행하였다.18-20 또한, 이마엽의 집행기능을 평가하기 위하여 통제 단어 연상 검사(Controlled Oral Word Association Test, COWAT), 숫자 부호화 검사(Digit Symbol Coding, DSC), 선 추적 검사(Trail Making Test, TMT), 한국판 색상 단어 스트룹 검사(Korean-Color Word Stroop Test, K-CWST)를 시행하였다.21-24

도파민작용제 투여

모든 환자에게 도파민작용제인 pramipexole(PMP)을 12주 동안 취침 한 시간 전에 투여하였다. 용량은 하루에 0.125mg으로 시작하였고, 2주가 지난 뒤 2배로 증량하였다. 효과와 순응도를 고려하여 투여시작 4주 이내에 적정용량으로 조절하였고, 적정화 후 일정량을 다음 신경심리검사 시행일까지 유지하였다. 수면설문지와 신경심리검사는 약물 투여 직전과 12~16주 사이에 두 차례 시행하였다.

통계 분석

치료 전후의 신경심리검사의 변화를 확인하기 위해 paired Student’s t-test를 이용하였고, 상관관계는 Spearman’s correlation analysis로 분석하였다. p-value가 0.05 이하일 경우 유의한 것으로 판정하였다.

결 과

인구학적 정보

Restless legs syndrome 환자들의 평균 나이는 49.7±11.4세이고 이 중 여자는 15명이다. 평균 교육 연수는 12.9±3.2년이었다. 체질량지수(body mass index)는 22.6±2.0 kg/m2, 혈청 ferritin은 107.1±78.3 ng/mL로 측정되었다. PMP 투여의 평균 추적 기간은 116.8±19.6일, 하루 투여량은 중앙값 0.19(0.13, 0.28) mg이었다.

수면 설문

Pramipexole 투여 후 IRLS, ISI, BDI, PSQI 점수가 투여 이전보다 유의하게 호전되었다(투여 전 vs. 투여 후; 29.5±5.3 vs. 14.6±9.8, p<0.001; 18.3±7.1 vs. 10.5±5.2, p=0.001; 18.1±11.2 vs. 11.4±8.3, p=0.002; 12.9±5.3 vs. 10.3±3.6, p=0.016). 그러나, ESS 점수는 유의한 차이를 보이지 않았다(Table 1).

신경심리검사

이마엽의 집행기능을 평가하는 COWAT phonemic word frequency, DSC 점수가 PMP 투여 후 의미 있게 호전되었다 (투여 전 vs. 투여 후; 36.5±12.6 vs. 41.5±12.6, p=0.03; 68.2±21.1 vs. 73.8±21.1, p=0.02)(Table 2). 시각 기억을 평가하는 RCFT immediate recall 검사에서도 의미 있는 호전을 보였다(17.72±7.84 vs. 20.13±7.33, p=0.05). Digit span forward, digit span backward, S-K-BNT, RCFT copy time, RCFT delayed recall, RCFT recognition, K-CVLT trial 1, KCVLT trial 1-5 total, K-CVLT short delay free recall, KCVLT long delay free recall, COWAT category word frequency: market, TMT-A, TMT-B, K-CWST color correct response검사에서는 호전되는 경향을 보였지만 유의한 차이는 보이지 않았다.

신경심리검사와의 수면관련 임상 요인과의 상관관계

신경심리검사 영역 중 의미 있는 호전을 보인 COWAT, DSC, RCFT 검사의 점수 변화와 수면 및 우울 검사의 점수 변화정도 간에 상관분석을 시행하였다. PMP 투여 후 COWAT phoneme word frequency total 점수, DSC 점수, RCFT immediate recall 점수의 호전과 IRLS, ISI, PSQI, BDI 점수의 호전 정도와는 유의한 상관관계를 보이지 않았다.

고 찰

본 연구에서는 도파민작용제인 PMP를 이전에 한 번도 치료받지 않은 중등도 이상의 RLS 환자에게 투여하였고, PMP가 RLS 증상의 중증도, 불면, 우울 및 수면의 질을 호전시킴을 확인할 수 있었다. 이는 이전의 여러 연구들과 일치하는 소견이다.6-9,25,26 인지기능에서는 집행기능 및 시각기억이 호전됨을 확인하였고, 이런 연구 결과는 도파민작용제가 RLS 환자에서 인지기능을 가역적으로 호전시킬 수 있음을 보여준다. 그러나 임상증상의 호전과 인지기능의 호전 사이에는 상관관계를 보이지 않았다.
Restless legs syndrome 환자에서는 인지기능이 저하되며, 주로 이마엽 영역에 해당하는 기능의 저하를 보인다.1,2,4,27 이마엽은 수면부족에 민감하게 영향을 받는 뇌 영역이고, RLS환자에서 수면부족이 흔하게 동반되므로, 주로 이마엽 영역의 인지기능이 RLS 환자에서 저하되는 것을 설명할 수 있다. 뇌 영상 및 신경생리검사를 이용한 연구들에서 RLS 환자의 이마엽 기능장애를 시사하는 결과가 보고되었다. 구조적 뇌영상 연구에서는 뇌들보(corpus callosum), 앞쪽 띠이랑(anterior cingulate gyrus), 중심앞이랑(frontocentral gyrus)의 백색질 부피가 RLS 환자에서 유의하게 감소되었고,28 기능적 뇌 영상 연구에서는 능동적으로 발의 발등굽힘과 발바닥쪽 굽힘을 시행할 때, 등가쪽 앞이마피질 부위의 활성이 RLS 환자에서 유의하게 증가되었다.29 그리고 안정 뇌파에서는, 이마엽의 베타 활성도가 증가되어 과각성상태와 유사한 소견을 보였다.5
본 연구에서 RLS 환자에서 도파민작용제 치료에 의하여 인지기능이 호전된 기전으로는 다음과 같은 3가지 가능성을 생각할 수 있다. 첫째, 도파민에 의해 수면부족, 주의력결핍과 같은 RLS 증상이 호전되었고, 이차적으로 인지기능이 호전되었을 가능성이 있다. 두 번째, 우울증상이 감소되면서 가성치매효과가 사라지고 이로 인해 인지기능이 호전된 것과같은 결과를 나타냈을 가능성이 있다. 마지막으로, 도파민 시스템의 조절장애가 RLS의 병태생리학적 원인으로 알려져있는데, 30 도파민의 투여가 불균형을 해소하여 도파민 시스템과 관련된 이마엽 인지기능을 호전시켰을 가능성이 있다.
인지기능의 호전 정도는 RLS 증상 개선, 수면장애 및 우울 증의 호전과는 의미 있는 상관관계를 보이지 않았다. 이 결과는 위에서 기술한 첫 번째 및 두 번째 가설을 지지하지 않는 소견이다. 그러므로 RLS 환자에서 도파민작용제 투여 후 인지기능의 호전은 다른 임상적인 변수들의 호전으로 인한 이차적인 결과가 아니고, 도파민이 직접적으로 인지기능에 영향을 미쳤을 가능성이 있다.
도파민은 뇌의 중요한 신경전달물질로서, 주로 운동계와 변연계의 기능을 조절하는 역할을 하고, 중대뇌변연계경로(mesocorticolimbic pathway)를 통해 인지기능에도 영향을 미친다.31 등가쪽 앞이마엽 및 뒤쪽 마루엽을 꼬리핵과 연결시키는 회로가 관련되어, 작업기억과 집행기능 등의 앞이마엽에 해당되는 인지 기능을 조절하는 역할을 한다.32-35 건강인과 파킨슨 환자를 대상으로 도파민작용제 또는 레보도파(levodopa)를 투여한 연구에서는, 약의 투여나 중단이 이마엽 인지기능에 관련된 집행기능과 작업기억을 변화시킨다고 보고하였다.31,33,34,36 도파민 투여는 시행하는 검사와 신경계 도파민의 농도에 따라 인지기능을 호전시키거나 오히려 악화시킬 수 있으며, 단순히 도파민의 부족이 인지기능을 저하시키는 것이 아니라 적절한 농도가 유지되지 않는 것이 인지기능 저하의 원인이다.37,38 본 연구에서도 PMP 투여가 도파민 불균형을 조절하여 인지기능의 호전이 나타났을 것으로 생각된다. 또한 집행기능과 시각기억이 호전된 점으로 미루어, RLS 환자에서 신경계의 도파민 농도가 감소되어 있음을 유추할 수 있다.30
본 연구에서는 대조군 없이 연구를 진행하여, RLS 환자에 서 도파민의 인지기능 호전 효과가 정상인에서 보일 수 있는 정도였는지 여부를 확인하지 못하였다. 그리고 연구를 진행한 환자의 숫자가 적어, 인지기능의 호전과 다른 임상적 변수와의 상관관계가 없다고 단정짓기에는 한계가 있다. 그럼에도 불구하고, 본 연구는 RLS 환자에서 도파민의 인지기능조절 효과를 보여주는 첫 논문으로, 도파민 치료가 RLS 환자에서 다른 증상뿐 아니라 인지기능의 호전도 초래할 수 있음을 보여준다는 점에서 의미가 있다고 하겠다.
